Lottery for COVID-19 Assistance


The City of Miami Beach has received Coronavirus Relief Funds from the Florida Housing Finance Corporation (FHFC) to provide emergency rent, mortgage, and maintenance/HOA fees assistance to households adversely impacted by COVID-19. Miami Beach households earning up to 120% Area Median Income ($58,440 for a household of one) can apply online via the City’s website for help with:

Rental assistance (including rent arrears, deposits and utility payments);
Mortgage payments (including principal and interest as well as insurance and property taxes paid via escrow only) and homeowner association fees. The assisted property may not have any current City liens and cannot be under any forbearance agreement in order to qualify for assistance.
Housing re-entry assistance such as security deposits, utility deposits and temporary storage of household furnishings.

Applications for the assistance lottery will be accepted beginning Thursday, October 29th at 9 a.m.

WHO is elegible for funds:
1. Applicants must be City of Miami Beach residents who have US Citizenship or permanent residency as demonstrated by birth certificate, naturalization certificate or valid (current) permanent residency card;
2. Qualifying utility costs to be paid may include: electricity, water and gas (bills must be in applicant’s name);
3. Program funds are only available to renters or homeowners that live or plan to relocate within City limits and must have valid Florida ID with the address of the assisted property;
4. Maximum household income must be at or below 120% of Area Median Income;
5. Applicant(s) must be certified by staff as income eligible and remain income eligible while receiving assistance;
6. Financial assistance shall be for a maximum of three (3) months, contingent upon applicant(s) becoming current on the expense once assistance is provided;
7. Rent/ mortgage must have been current as of March 12, 2020;
8. Applicants cannot receive funds from another non-profit or governmental jurisdiction whose funds came from a federal source (Applicants will be required to execute a Non-Duplication of Benefits Affidavit).
HOW we determine eligibility:
• The number of people residing in the household including name, age, relationship to head of household, current address and home phone number;
• Name and address of employer(s), work phone number(s), position title and number of years on job with the current employer;
• Sources of annual income, including earned, unearned and asset income, and a statement signed by all of the adults who reside in the household consenting to the disclosure of information for the purpose of verifying income and assets in order to determine income eligibility for program assistance;
• A signed statement indicating that the applicant understands that all information provided is subject to Florida’s public records laws;
• A statement that it is a first-degree misdemeanor to falsify information for the purpose of obtaining assistance.
COVID-19 Rent Assistance Required Documentation:
-Identification

The applicant must prove his identity.

.Florida-issued Driver’s License or Identification Card (valid and reflecting the address for which the applicant is seeking rent assistance)

.Social Security card for applicant (listing name that matches government-issue identification provided)

-US Citizenship/Residency

Federal funds require citizenship or residency for receipt.

.U.S. birth certificate

.U.S. citizenship or residency (certificate, card or valid passport)

-Miami Beach Residency

Only Miami Beach residents can be served with entitlement funds.

.Valid residential lease listing applicant(s) as party/lessee

.Current utility bill listing applicant(s) as customer (FPL, TECO, cable, etc.)

-Compliance prior to COVID-19 emergency declaration

Applicants must have been current with rent and utility payments prior to March 1, 2020

.Evidence of employment on March 1, 2020 (payroll records, direct deposit records, etc.)

.Evidence of current rent as of March 1, 2020 (rent receipt, cancelled check, etc.)

.Evidence of current utility payments as of March 1 ,2020 (cancelled checks, wire transfers, online statements, etc.)

-Evidence of adverse financial impact by COVID-19

Applicant must demonstrate that he/she has been adversely impacted financially as a direct result of COVID-19

.Loss of employment (WARN letter, unemployment application, etc.)

.Reduction in income (furlough letter from employer, payroll records, bank statements, etc.)

.3-Day/5-Day Notice of Eviction (must be dated within 30 days of application for assistance and indicate the period in arrears and past due rent amounts exclusive of all non-rent fees)

.Past Due Statements (utilities) dated within 30 days of assistance date

-Income Eligibility

Federal and State income guidelines serve primarily low-income residents earning less than 120% Area Median Income ($6,400 per month for a household of 1) and with limited assets.

.Bank statements (February 1, 2020 to application date)

.Payroll records (must include March 1, 2020 to application date)

.Benefits letter (Social Security Administration, pension, etc.)

.2019 tax return

-Landlord Participation

Landlord must consent to receiving payment from City to bring applicant(s) current on rent

.Online vendor registration with City

.Completion of Landlord Participation Agreement

I/We understand that by completing this on-line form and receiving a registration confirmation number I am not guaranteed any form of rent assistance or service. I/ We understand that inclusion in the lottery waitlist does not mean your application is confirmed eligible or that the applicant will receive assistance. I/ We understand that if I am selected via the lottery process and contacted by the Office of Housing and Community Services, I will need to provide supporting documentation outlined as program eligibility requirements for processing as required by program guidelines. I/ We understand that if I am unable to produce supporting documentation within 48 hours, staff will move on to the next applicant on the list, in numerical order. I/ We understand that because program funding is limited, not all persons on the lottery waitlist will be contacted. I understand that staff will contact persons in the order of the lottery until all funds are exhausted. Applicant(s) understand(s) that Florida Statute 817 provides that willful false statements or misrepresentation concerning income and assets or liabilities relating to financial condition is a misdemeanor of the first degree and is punishable by fines and imprisonment provided under S775.082 or 775.83 or 775.084.
